Annual Report of Activities - Faculty Members. 11.60.10 Purpose - Faculty Members’ annual reports, and the Xxxx’x responses, are to be used principally to encourage growth in Members’ teaching ability and scholarly contribution. Such reports constitute formative, not summative, evaluations, intended to assist Members in their professional development goals.

The Chairperson shall summarize any comments that may be received and submit them to the Xxxx together with his or her own comments and two copies of the reports by November 30. The Xxxx shall forward one copy of all annual reports for his/her faculty together with the Xxxx’x comments to the Academic Vice-President. A copy of the Xxxx’x comments shall be sent to each Faculty Member and to his or her Chairperson. The Annual Report shall include a statement of activities for the previous September 1 to August 31 period. The Annual Report and the written feedback shall be kept in the Faculty member’s official file.

(a) Each Faculty Member shall submit an electronic copy of an annual report of Activities to their Chairperson, by October 15th of each year with the exception of probationary appointees who shall report in compliance with 10.1.12 and Faculty Members on approved leave who shall submit their annual report no later than thirty (30) days upon return from approved leave. The Chairperson shall make a copy immediately available electronically to all Department members and invite them to review and comment on the reports by November 15. The Chairperson shall summarize any comments that may be received and submit them to the Xxxx together with their own comments along with a copy of the reports by November 30. The Xxxx shall forward one copy of all annual reports for their faculty together with the Xxxx’x comments to the Vice President, Academic and Research. A copy of the Xxxx’x comments shall be sent to each Faculty Member and to their Chairperson. The Annual Report shall include a statement of activities for the previous September 1 to August 31 period excepting for periods of approved leave. The dates of the leave period shall be stated in the Annual Report. The Annual Report and the written feedback shall be kept in the Faculty member’s official file.
