Annual Report of Activities. Each Librarian Member shall submit to the University Librarian, copied to the supervising Associate University Librarian where appropriate, an Annual Report of Activities by May 5th of each year. The Annual Report shall include a statement of activities for the period from the previous May 1 to April 30. The Annual Report shall be copied to the Member’s Official File. The Annual Report, which shall be completed on a standardized form supplied by the University Librarian, shall include the following information:

If (b) and/or (c) are not part of a Member’s assigned duties and responsibilities, the Member may include such activities in the Annual Report if they wish to do so.
