Antivirals. Avoid combination therapy containing simeprevir and sofosbuvir due to risk of severe bradycardia and heart block; Unless other antiarrhythmics cannot be given • Grapefruit juice inhibits cytochrome P450 3A4 and may increase the plasma concentration of amiodarone. Grapefruit juice should be avoided during treatment with oral amiodarone • Laxatives (stimulant): Stimulant laxatives e.g. Bisacodyl, Senna may cause hypokalaemia and increase the risk of torsades de pointes. Concomitant use with amiodarone are cautioned: • Anticoagulants: Warfarin clearance is reduced. This can lead to sudden and pronounced increase in INR. Interaction reaches its peak in 6-7 weeks. Reduce warfarin dose and monitor weekly, tailoring the warfarin dose to the INR target. • Amiodarone increases the exposure to dabigatran thus increase the risk of bleeding. Adjust the dosage of dabigatran according to manufacturers’ advice (note dose may be different for different clinical indications). • Cardiac Glycosides: Plasma level of Digoxin approximately doubles over weeks after commencement of amiodarone. Reduce dose and monitor digoxin level if applicable • Beta-blockers: Potentiation of negative chronotropic properties and conduction slowing effects may occur. • Calcium channel inhibitors: Rate lowering calcium channel inhibitors e.g. Diltiazem, verapamil potentiates negative chronotropic properties and conduction slowing effects may occur (under specialist advice only) • Ciclosporin: Plasma level of ciclosporin increased when used with amiodarone. Manufacturer advises monitor concentration and adjust dose. • Drugs that may cause hypokalaemia/ hypomagnesaemia / hypokalaemia/ hypomagnesaemia: Caution should be exercised over combined therapy with the following drugs which may also cause hypokalaemia: and/or hypomagnesaemia e.g. diuretics, systemic corticosteroids, tetracosactrin, IV amphotericin, aminophylline, theophylline • Statins: the risk of muscular toxicity (e.g. Rhabdomyolysis) is increased by concomitant administration of amiodarone with statins metabolised by CYP 3A4 such as simvastatin, atorvastatin, and lovastatin. It is recommended to use a statin not metabolised by CYP 3A4 when given with amiodarone. • Phenytoin: Plasma level of phenytoin increased with amiodarone.   • Influenza Vaccination The parties agree that influenza vaccinations may be beneficial for patients and employees. Upon a recommendation pertaining to a facility or a specifically designated area(s) thereof from the Medical Officer of Health or in compliance with applicable provincial legislation, the following rules will apply:

  • Influenza Vaccine Upon recommendation of the Medical Officer of Health, all employees shall be required, on an annual basis to be vaccinated and or to take antiviral medication for influenza. If the costs of such medication are not covered by some other sources, the Employer will pay the cost for such medication. If the employee fails to take the required medication, she may be placed on an unpaid leave of absence during any influenza outbreak in the home until such time as the employee has been cleared by the public health or the Employer to return to the work environment. The only exception to this would be employees for whom taking the medication will result in the employee being physically ill to the extent that she cannot attend work. Upon written direction from the employee’s physician of such medical condition in consultation with the Employer’s physician, (if requested), the employee will be permitted to access their sick bank, if any, during any outbreak period. If there is a dispute between the physicians, the employee will be placed on unpaid leave. If the employee gets sick as a reaction to the drug and applies for WSIB the Employer will not oppose the application. If an employee is pregnant and her physician believes the pregnancy could be in jeopardy as a result of the influenza inoculation and/or the antiviral medication she shall be eligible for sick leave in circumstances where she is not allowed to attend at work as a result of an outbreak. This clause shall be interpreted in a manner consistent with the Ontario Human Rights Code.

  • Hepatitis B Vaccine Where the Hospital identifies high risk areas where employees are exposed to Hepatitis B, the Hospital will provide, at no cost to the employees, a Hepatitis B vaccine.

  • Devices BNY Mellon will restrict the transfer of Customer Data from its network to mass storage devices. BNY Mellon will use a mobile device management system or equivalent tool when mobile computing is used to provide the services. Applications on such authenticated devices will be housed within an encrypted container and BNY Mellon will maintain the ability to remote wipe the contents of the container.

  • MEDICALLY FRAGILE STUDENTS 1. If a teacher will be providing instructional or other services to a medically fragile student, the teacher or another adult who will be present when the instruction or other services are being provided will be advised of the steps to be taken in the event an emergency arises relating to the student's medical condition.
