Medications. Psychotropic medications and medications associated with treating a diagnosed mental health condition.
Medications. Management agrees that the administration of medications shall be conducted in compliance with state regulations and applicable State Practice Acts. Management shall enforce state laws concerning the administration of medications.
Medications. Information about medications the beneficiary has received, or is receiving, to treat mental health and medical conditions, including duration of medical treatment. The assessment shall include documentation of the absence or presence of allergies or adverse reactions to medications, and documentation of an informed consent for medications;
Medications. Unless otherwise set forth in the student’s ISP, CONTRACTOR shall comply with the requirements of California Education Code section 49423 when CONTRACTOR serves a LEA student that is required to take prescription and/or over-the-counter medication during the school day. CONTRACTOR may designate personnel to assist the LEA student with the administration of such medication after the LEA student’s parent(s) provides to CONTRACTOR: (a) a written statement from a physician detailing the type, administration method, amount, and time schedules by which such medication shall be taken; and (b) a written statement from the LEA student’s parent(s) granting CONTRACTOR permission to administer medication(s) as specified in the physician’s statement. CONTRACTOR shall maintain, and provide to LEA upon request, copies of such written statements. CONTRACTOR shall maintain a written log for each LEA student to whom medication is administered. Such written log shall specify the student’s name; the type of medication; the date, time, and amount of each administration; and the name of CONTRACTOR’s employee who administered the medication. CONTRACTOR maintains full responsibility for assuring appropriate staff training in the administration of such medication consistent with physician’s written orders. Any change in medication type, administration method, amount or schedule must be authorized by both a licensed physician and parent.
Medications. 18.9.1 If Contractor provides or stores medications, the Contractor shall store and monitor medications in compliance with all pertinent statutes and federal standards.
18.9.2 Contractor shall have written policies and procedures regarding the use of prescribed medications by clients, and for monitoring and storing of medications.
18.9.3 Prescription and over the counter medications which expire and other bio-hazardous pharmaceutics including used syringes or medications which are not removed by the client upon termination of services shall be disposed of by the program director or a designated substitute, and one other adult who is not a client. Both shall sign a record, to be retained for at least one year.
18.9.4 Contractor shall have at least one program staff on duty at all times trained to adequately monitor clients for signs and symptoms of their possible misuse of prescribed medications, adverse medication reactions and related medical complications.
Medications. Children will not be given any over the counter or prescription medication by our staff. Parents/Guardians must make necessary arrangements to administer prescribed and/or over the counter medications themselves.
Medications. Please list all medications currently prescribed for the dog(s).
Medications. In certain situations, an employee’s use of medication can pose a risk to the safety of the employee or to others. If an employee’s use of any medication could adversely affect the safety of the employee, co-workers, students or members of the public; the employees job performance; or safe or efficient District operation, then the employee must provide his/her supervisor with a physician’s notice that specifies any on-duty-related limitations resulting from use of the medication. The knowing failure to provide such notice of work limitation may subject the employee to disciplinary action.
Medications. List any ongoing medications. Include any medications given at home if they might impact the child's health while in child care (seizure, cardiac or asthma medications, etc.). Short-term medications such as antibiotics do not need to be listed on this form. Long-term antibiotics such as antibiotics for urinary tract infections or sickle cell prophylaxis should be included. Medications. CONTRACTOR shall distribute medications only when it has obtained an original, written statement from the pupil’s physician detailing the type, dose, frequency and method of administration and written permission from the pupil’s parent to dispense the prescribed medication. CONTRACTOR shall keep a written log delineating date, time, dose, type of medication, method of administration, and the name of CONTRACTOR’s Personnel who dispensed medication to the pupil. CONTRACTOR shall not make any change in medication, dose, frequency or method of administration without the advance written authorization of a licensed physician and the pupil’s parent.
