Application for donation. Applications for catastrophic illness or injury sick leave donation must be submitted to the Employer in writing. Applications will include, but not be limited to, the following information: A. The nature of the claimed catastrophic illness or injury; B. Physician(s) diagnosis and prognosis of the catastrophic illness or injury; C. Projected date of return to duty; D. The Employer will provide the applicant's sick leave usage record; the Employee may provide an explanation of previous leave usage; and E. Any other pertinent information the applicant wishes to submit to the Sheriff for his consideration.
