Application Requirements for Long Term Disability. Where after three (3) months’ absence due to illness, a regular staff member’s physician considers recovery from illness not likely or uncertain by the end of a total of six (6) months’ sick leave, the staff member shall also make an application for Long Term Disability Insurance Benefits (see Article 18.06) at the end of a total of three (3) months’ absence. This application shall be made by the staff member through the Benefits Office in the Human Resources Department.
