APPOINTMENT/REAPPOINTMENT INFORMATION. The Foundation shall provide each employee on or about their initial appointment date with a copy of their Personnel Action Form (PAF), which shall include the employee’s appointment start date and end date, rate of pay, hours per pay period, project number, Foundation job title and Project Director and/or Principal Investigator.
