Common use of Assessment Clause in Contracts

Assessment. 11.1. CONTRACTOR shall ensure that all client medical records include an assessment of each client's need for mental health services. 11.2. CONTRACTOR will utilize the seven uniform assessment domains and include other required elements as identified in BHIN 22-019 and document the assessment in the client’s medical record. 11.3. For clients aged 6 through 20, the Child and Adolescent Needs and Strengths (CANS), and for clients aged 3 through 18, the Pediatric Symptom Checklist-35 (PSC-35) tools are required at intake, every six months during treatment, and at discharge, as specified in DHCS MHSUDS INs 17-052 and 18-048. 11.4. The time period for providers to complete an initial assessment and subsequent assessments for SMHS are up to clinical discretion of COUNTY; however, CONTRACTOR’S providers shall complete assessments within a reasonable time and in accordance with generally accepted standards of practice.
