Care Coordination for Special Populations. The CCN shall implement mechanisms to assess each Medicaid enrollee identified as having special health care needs in order to identify any ongoing special conditions of the enrollee that require a course of treatment or regular care monitoring. The assessment mechanisms must use appropriate health care professionals. The CCN shall have mechanisms to assess the quality and appropriateness of care furnished to enrollees with special health care needs. The CCN shall identify members with special health care needs within ninety-(90) days of receiving the member’s historical claims data (if available).
