Care Rendered by More Than One Provider. If a Member transfers from the care of one Provider to another Provider during treatment, or if more than one Provider renders services for one dental procedure, Blue Cross of Idaho will pay no more than the amount that it would have paid had but one Provider rendered the service.

  • Time Limits to Present Initial Grievance An employee who wishes to present a grievance at Step 2 of the grievance procedure, in the manner prescribed in Article 9.4, must do so not later than thirty (30) days after the date:
