Common use of Certification of Sick Leave Clause in Contracts

Certification of Sick Leave. The Chief or the Board of Trustees may require evidence as to the adequacy of the reasons for any member's absence during the time which sick leave is requested, including a certificate from a licensed practitioner verifying proper use of sick leave. After each thirty (30) days of sick leave, a member must submit to the Chief a certificate from a licensed practitioner verifying the proper use of sick leave use and providing an estimated date of return to work, if known. In the event an employee has been provided documentation from their physician indicating that the employee cannot return to work for a period of time greater than 30 calendar days, the requirement will be waived and an update of the employee’s condition will be required at the time of the next scheduled examination.
