Chronic Illness. In the event the Policy(ies) provides for such option through a Chronic Illness Rider and Insured satisfies the Policy requirements regarding what it means to be Chronically Ill, then Insured must satisfy at least one of the following in order to have the right to request (in writing) and receive amounts pursuant to the Chronic Illness Rider: (i) Insured has not Separated From Service, OR (ii) Insured Separates From Service on or after November 1, 2026, for any reason other than a Termination For Cause, OR (iii) Insured Separates From Service on or after a Change in Control for any reason other than a Termination For Cause, OR (iv) Insured Separates From Service prior to November 1, 2026, due to a Termination Without Cause, OR (v) Insured Separated From Service under a qualifying retirement where qualifying retirement is defined as the Insured’s combined total of age plus service (both in whole years) being greater than or equal to Seventy-Five (75), OR (vi) Insured Separates From Service as a result of becoming Disabled. In the event the forgoing requirements are satisfied, then subject to any further limitation on amounts imposed by the Policy(ies) itself, Insured shall have the right to request (in writing) and receive from the Policy(ies), an amount which does not exceed Five Hundred Thousand Dollars ($500,000).
Chronic Illness. If the insured is certified (within the last 12 months) by a licensed health care practitioner as : a) being unable to perform (without substantial assistance from another individual) at least two activities of daily living (ADL) for a period of at least 90 days due to a loss of functional capacity, b) having a level of disability similar to the level of disability described in (a), c) requiring substantial supervision to protect such individual from threats to health and safety due to severe cognitive impairment. The benefit payable shall be computed by multiplying the total Accelerated Benefits Rider payout by the ratio of the reinsured "net amount at risk" to the face amount (Death Benefit) used in the total Accelerated Benefits Rider payout calculation. SCHEDULE IV - REINSURANCE PREMIUMS (CONTINUED This Rider allows a chronically ill insured to accelerate benefits after the first two policy years. The maximum payout under the Accelerated Benefits Rider shall be limited to $1,000,000. The Reinsurer shall pay its share (based on the percentage of the base policy reinsured with the Reinsurer) of the discounted benefit in a lump sum. There is no additional premium for this Rider. The Ceding Company will continue to pay reinsurance premiums on the unaccelerated portion of reinsured business until the policy is terminated or the date of death of the insured, unless a reinsured waiver of premium benefit applies. ACCELERATED CARE RIDER The premiums payable for the Accelerated Care Rider on Traditional Life policies shall be based on the appropriate point-in-scale life rate from the attached Rate Table labeled C-6 (for "Lifetime" pay without nonforfeiture benefit), multiplied by the applicable pay percentage as shown below: POLICY YEAR ALLOWANCE
Chronic Illness. A chronic illness is one in which bodily health impairment is constant and consistent and of long duration.
Chronic Illness. 2.2.1 Costs of treating the unexpected worsening of a chronic illness, up to 5,000 Euros. 2.2.2 A chronic disease shall be understood as a state of health which is by its nature recurrent, constant or long-term.

For a student to be given accommodations, he/she must complete the chronic illness form and submit the proper medical documents to the school nurse who will then provide copies to the ▇▇▇▇ of Students and the Counseling Department for approval. Chronic Illness. 6.9.1. AIA recognises that an employee with a chronic illness may need: a. Flexible work arrangements; and or b. Extended unpaid leave. 6.9.2. AIA and an employee will develop an individual plan that accommodates and recognises the needs of the employee and the employees team or work area. AIA will consider the employee’s capacity to work in consultation with the employee’s health care professional. 6.9.3. AIA may grant up to 12 months unpaid personal leave.
Chronic Illness. Medical Leave In the event of a chronic or acute illness, automobile accident, etc., the Company shall provide employees with up to three (3) paid days per calendar year. Employees may use chronic illness/medical leave benefits due to their own illness or injury of that of a child, spouse or parent.

  • Illness injury, or pregnancy-related condition of a member of the employee’s immediate family where the employee’s presence is reasonably necessary for the health and welfare of the employee or affected family member;

  • Critical Illness Three (3) days per year, with pay, shall be granted in the case of a critical illness or accident to a member of the employee's immediate family as defined in Section 9.4.2. A statement by the physician verifying the need for the employee to be present with the immediate family member shall be attached to the absence form.

  • Sickness In the event that an employee requires hospitalization or is seriously ill during his/her annual vacation period, the employee shall, upon request and upon presentation of a physician's statement, apply the period of illness or hospitalization to sick leave rather than vacation, provided the sick bank is not used. The employee must inform the College of the claim within one (1) week of returning to work.

  • Critical Illness Leave (i) An Employee who has completed at least ninety (90) days of employment, and is a family member of a critically ill child or a critically ill qualified adult relative, is entitled to leave of absence without pay or benefits: • for a period of up to thirty-six (36) weeks to care for their critically ill child; or, • for a period of up to sixteen (16) weeks to care for a critically ill qualified adult relative.

  • SICKNESS ABSENCE Absence Due to Sickness or Quarantine Prior to the Eighth Full Calendar Day of Absence 29.01 An employee having six (6) months net credited service, or more, who is absent on account of sickness or quarantine, shall be paid for continuous absence prior to the eighth full calendar day of such absence, as follows: (a) An employee with six (6) months but less than two (2) years net credited service shall be paid for that part of the absence in excess of four (4) consecutive half tours. (b) An employee with two (2) but less than four (4) years net credited service shall be paid for that part of the absence in excess of two (2) consecutive half tours. (c) In the determination of pay treatment in Subsections 29.01 (a) and (b), a return to work not exceeding two (2) half tours shall not be considered to have interrupted the continuity of the absence, nor the consecutiveness of the half tours of absence. However, for purposes of determining the eighth full calendar day of absence, any return to work shall interrupt the continuity of an absence. (d) An employee with four (4) or more years net credited service shall be paid for the full absence. (e) An employee is not entitled to any pay or other benefit provided under this Article for any day in which she is in receipt of, or entitled to, any pay or other benefit under any other provision of this Agreement. 29.02 Upon the eighth full calendar day of an absence covered under Section 29.01, such an absence shall be treated in accordance with applicable Company practices currently in effect, or as amended from time to time following notification to the Union.
