Clinical Laboratory a medical laboratory licensed where required, performing within the scope of such licensure, and is not affiliated or associated with a Hospital or Physician.
Clinical Laboratory. Improvements Act (CLIA) certification or waiver. As applicable, if Provider performs any laboratory tests on human specimens for the purpose of diagnosis and/or treatment, Provider agrees to acquire and maintain the appropriate CLIA certification or waiver for the type of laboratory testing performed. Provider further agrees to provide a copy of the certification if requested by Subcontractor or Health Plan. A State authorized license or permit that meets the CLIA requirements may be substituted for the CLIA certificate pursuant to State law. Medicare and Medicaid programs require the applicable CLIA certification or waiver for the type of services performed as a condition of payment. Provider must include the appropriate CLIA certificate or waiver number on claims submitted for payment for laboratory services.
Clinical Laboratory. Improvements Act (CLIA) certification or waiver. In addition to the requirements set forth in Section 9.14, if Provider performs any laboratory tests on human specimens for the purpose of diagnosis and/or treatment, Provider must include the appropriate CLIA certificate or waiver number on claims submitted for payment for laboratory services.
Clinical Laboratory. In the event Dental Office has a need to use the services of a clinical laboratory for services rendered to a LIBERTY Member, then Dental Office shall use a Medicare Certified Independent Laboratory or Medicare Certified Hospital Laboratory.
Clinical Laboratory. Clinical laboratory (eg, hematology, clinical chemistry, coagulation, and urinalysis) values will be evaluated for each laboratory parameter as appropriate. Abnormal laboratory values will be flagged and identified as outside (above or below) the normal range. Reference (normal) ranges for laboratory parameters will be provided. Descriptive summary statistics (eg, n, mean, SD, median, minimum, and maximum for continuous variables; n [%] for categorical variables) for laboratory parameters and their changes from baseline will be calculated. Laboratory values will be summarized by visit.
Clinical Laboratory. Copies of the HCFA-116 form and Clinical Laboratory Improvement Amendments (CLIA) Registration Certificate or CLIA Certificate of Accreditation or Compliance. END-STAGE RENAL DISEASE (ESRD) FACILITY: A copy of Medicare’s Certification Letter. HOME HEALTH AGENCY & SUBUNIT(S): A copy of Medicare’s Certification letter, and a letter stating the applicant’s fiscal year end. HOSPICE: A letter stating the applicant’s fiscal year end. INDEPENDENT PHYSICAL THERAPIST/REHABILITATION AGENCY: A copy of Medicare’s certification letter. MATERNAL/INFANT SUPPORT AGENCY: A copy of the approved Michigan Department of Community Health Maternal/Infant Support Services Provider Application. MEDICAL SUPPLIER (DME): A list or brochure describing the supplies/equipment that you will provide and a copy of your state license or state sales tax license.
Clinical Laboratory. Individual assignments involving clinical laboratory will normally be an annual average of one-hundred-five (105) to one-hundred-eleven (111) contact hours.
