Communication of Request. If conditions at an Evacuating Facility are unsafe, an Evacuating Facility may request a resident transfer to another Participating Facility through the RHCC and on-scene incident commander, if applicable. Written documentation is required prior to the actual transfer of a resident or if a technological failure prevents this, as soon as feasible thereafter.
Communication of Request. The request for the transfer of personnel can initially be made verbally and should be followed by written or Web Emergency Operations Center (Web EOC) documentation of the request. [Web EOC is an internet-based documentation and communication tool used by multiple systems throughout the County of San Diego collaboratively with the MOC (Medical Operations Center) and EOC (Emergency Operations Center)]. Web EOC will be used primarily by area coordinators to facilitate information provision and updates. Requests will be made in a standardized format. A request and documented response will occur prior to the arrival of personnel at the recipient facility. The recipient facility will identify to the donor facility the following: ▪ The type and number of requested personnel ▪ An estimate of how quickly the request is needed ▪ The location where they are to report ▪ An estimate of how long the personnel will be needed
Communication of Request. Initial communications of need shall be made through the Healthcare Coalition via AIMS or other appropriate means of communication. Affected Entity’s request to Assisting Entities for transfer of supplies or equipment initially may be made verbally. The request, however, must be followed up with electronic or written communication. The electronic or written request should be submitted to the Assisting Entity
a. Indicate the quantity and exact type of resources;
b. Estimate how quickly the request must be filled;
c. Note the time period for which the resources will be needed;
d. Specify the location where the resources should be delivered.

This should ideally occur prior to the arrival of personnel at the affected hospital. The affected hospital will identify to the assisting hospital/RHRC the following:
a. The type and number of requested healthcare personnel.
b. An estimate of how quickly the request is needed.
c. The location where they are to report.
d. An estimate of how long the personnel will be needed.
Communication of Request. The request for the transfer of pharmaceuticals, medical supplies, or equipment initially can be made verbally. The request, however, must be followed up with a written communication. This should ideally occur prior to the receipt of any material resources at the recipient hospital. The affected hospital will identify to the assisting hospital/RHRC the following:
a. The quantity and exact type of requested items.
b. An estimate of how quickly the request is needed.
c. Time period for which the supplies will be needed.
d. Location to which the supplies should be delivered. The assisting hospital will identify how long it will take to fulfill the request.
Communication of Request. The request for the transfer of patients initially can be made verbally to the appropriate MRCC (emergent evacuation) or RHRC (urgent evacuation – allows time to set up RHRC). The request, however, must be followed up with a written communication as soon as possible. The affected hospital will identify for MRCC and appropriate local EMS agencies / RHRC
a. The number of patients needed to be transferred.
b. The general nature of their illness or condition.
c. Any type of specialized services required

The undersigned will agree to the best of our abilities, the staff from the evacuating facility will remain with the consumers during the transfer and for the extent of the disaster. Some information that may need to be communicated includes but is not limited to: • The type and number of requested personnel • An estimate of how quickly the request is needed • The location where they are to report • An estimate of how long the personnel will be needed

Affected Entity’s request to Assisting Entities for transfer of supplies or equipment initially may be made verbally. The request, however, must be followed up with written communication such as HEICS 201. The written request must be submitted to the Assisting Entity prior to the receipt of any material resources at the Affected Entity. The Affected Entity will identify to the Assisting Entity the following:
a. Indicate the quantity and exact type of resources;
b. Estimate how quickly the request must be filled;
c. Note the time period for which the resources will be needed;
d. Specify the location where the resources should be delivered.
