DOCUMENTATION AND COMMUNICATION. ‌ 4.1. You will pass on immediately to Us or the Applicant as applicable, without amendment (unless otherwise agreed by Us), any documentation which is either supplied by Us for the benefit of or completion by the Applicant or provided by the Applicant in relation to the Application. You will advise Us of all material facts known by or divulged to You in relation to business undertaken or to be undertaken with Us. 4.2. Without limiting your other obligations, You will pass on to Us all the necessary information and independently verifiable evidence that the Applicant needs to provide to enable Us to assess affordability of the Mortgage. 4.3. You acknowledge that We may send communications directly to the Applicant. 4.4. All books and documents, computer hardware and software and any other items of property belonging to Us and in Your possession or under Your control must at all times be available to Us for inspection and be delivered in good condition (allowing for fair wear and tear) to Us on demand. All material supplied by Us shall remain Our property and You will only use such materials for the sole purpose of fulfilling Your obligations under these Terms. 4.5. You will not produce or distribute any documentation (unless supplied by Us for distribution) containing Our name, logos or Our trade marks without Our prior written consent. 4.6. You will ensure that Our latest literature (as provided by Us to You from time to time) is used and You will destroy out of date stocks. 4.7. Other than by using the standard material supplied by Us for the purpose of performing Your obligations under these Terms, You must not publish, circulate, issue or release any advertisement or literature relating to Our business or Us or make use of Our name or logo or that of any of Our subsidiaries or affiliates or associated companies without previous written authorisation from Us. 4.8. You will not sign or amend any documents or policies on Our behalf nor make any statements or promises or representations of any kind whether written or oral which bind or purport to bind Us. You will not hold Yourself out as having authority to make any such representation. 4.9. Where You request Us to provide You with data relating to Your Applicants and book of business, including for the purposes of reconciling the data We hold with the data You hold, We reserve the right to charge You for Our reasonable administrationcosts incurred in providing this data. 4.10. DOCUMENTATION AND COMMUNICATION. 1. The Collaborating Physician shall be responsible for documenting the communication in the patient medical record maintained by the Collaborating Physician. The Collaborating Physician shall, within 24 hours, communicate to the Collaborating Pharmacist any changes initiated to a patient's drug therapy that is subject to an Agreement; a written, telephonic, or electronic prescription which contains specific dosage information may satisfy this requirement. The collaborating pharmacist shall, within 24 hours, communicate to the collaborating physician and any physician(s) designated by the patient any changes to a patient’s drug therapy and/or individual patient care services as set out in the Agreement. The Agreement shall describe the methods for documenting the patient medical record by the Collaborating Pharmacist and the Collaborating Physician, for documentation of services performed pursuant to the Agreement, and for communication and feedback between the Collaborating Pharmacist and the Collaborating Physician. All such records shall be maintained by the Collaborating Physician for a period of not less than six (6) years from the date of the last patient contact, or if the patient is a minor, the record shall be maintained for a period of not less than eight (8) years from the date of the last patient contact. All such records shall be maintained by the Collaborating Pharmacist within the employing pharmacy for a period of not less than two (2) years from the date of the last patient contact. 2. A pharmacist performing testing or screening for and treatment of acute, uncomplicated illness or injury conditions shall also: (i) Provide each patient with written information to advise the patient to seek follow-up care from either a physician with whom the patient has a pre-existing relationship, or, if no pre-existing relationship exists, to seek follow-up care from the collaborating physician. (ii) Provide the patient with a consent form allowing any changes to a patient's drug therapy and/or individual patient care services as set out in the Agreement to be provided to the collaborating physician and the physician with whom the patient has a pre-existing relationship.

The Client represents, warrants, and covenants to Cidel that the information contained in any forms or documents provided by the Client are true and correct as of the execution date of this Agreement. d. The Client shall notify Cidel immediately if it anticipates that any representation or warranty made by the Client herein ceases to be correct. e. The Client acknowledges that Cidel has no duty or obligation to act upon instructions provided by the Client through email, regular mail, or fax, unless the instructions are explicitly confirmed and acknowledged by Cidel. f. The Client understands and agrees that Cidel may send communications to the Client at any address including a mailing address or email address, or fax number provided to Cidel by the Client in writing. The Client acknowledges that all communications sent to any email address, mailing address, or fax number provided by the Client will be considered to be delivered personally, whether the Client actually receives the communication or not . g. The Client acknowledges that all communications, including account statements will be considered final unless the Client notifies Cidel in writing within 30 days. DOCUMENTATION AND COMMUNICATION. 4.1 The Offeror agrees: 4.1.1 to provide promptly to the Company all such information about itself, its Group and its directors as may be reasonably requested and which is required by the Company (having regard to the Code and applicable regulations) for the purpose of 4.1.2 to provide all other assistance which may be reasonably required for the preparation of the Scheme Document including access to the relevant professional advisers; 4.1.3 to procure that the relevant persons accept responsibility for all information in the Scheme Document relating to the Offeror, its Group and its directors; 4.1.4 that, if any supplemental circular or document is required to be published in connection with the Scheme or any variation or amendment to the Scheme, it shall promptly provide such co-operation and information necessary to comply with all regulatory provisions as the Company may request in order to finalise such document. 4.2 The Offeror shall: 4.2.1 allow the Company reasonable opportunity to provide comments on: (i) any announcement or communication to Company Shareholders or the Company’s optionholders or bondholders; and (ii) any material communication to the Company’s financiers, joint venture partners or major tenants, in each case, in connection with the Acquisition, and the Offeror shall take into account the Company’s reasonable comments on such announcement or communication (as applicable); 4.2.2 give the Company reasonable prior notice of any meetings or calls with Company Shareholders or the Company’s optionholders, bondholders, financiers, joint venture partners or major tenants in connection with the Acquisition, consult with the Company in respect of the purpose of any such meetings or calls and, where reasonably requested, allow the Company and/or its advisers to attend any such meetings or calls and, if the Company and/or its advisors do not attend any such meetings or calls, promptly provide the Company with a written summary of the materials items discussed at such meetings or on such calls; and 4.2.3 not communicate with the Company’s employees (including holding any meetings or calls with, or publishing any announcement directed at, the Company’s employees), without the prior written consent of the Company.

Communication with parents and the District will occur via email and through phone calls based on the availability of Community Behavior Consulting personnel. All forms which are supplied by Mid Atlantic at LHCSA's option must comply with all applicable State and Federal regulations and conform to LHCSA's plans of treatment and care as established by LHCSA. Mid Atlantic will furnish copies of such documentation to the LHCSA within ten (10) business days of providing the services under this agreement. Telephone consultation with the LHCSA as needed to report any changes in the Patient's condition or circumstances, and to keep LHCSA's personnel appropriately informed of the patient's progress and/or problems which may arise.

All AMIDEAST’s marketing products, such as pop-ups, roll-ups, posters, folders, binders and on-line materials will utilize the image. LTD’s public relations will focus its outreach to its main constituents, including teachers, principals district and ministry staff, USAID, teacher educators, international and local NGOs that work in the field of Education, and members of community organizations. Given the size and number of beneficiaries, an online newsletter will be published quarterly. The newsletter will consist of a series of brief articles about recent LTD achievements, activities, and events. It will include interviews with program participants and stakeholders, as well as articles written by other MoEHE public relations officers. The newsletter will be published in both Arabic and English. The AMIDEAST LTD website will be a venue for internal and external communication. It will provide information to visitors, partners, and participants about all aspects of the program. Content will include articles, pictures, activities, and notices of upcoming events. On a routine basis, LTD program managers and the Communications Officer will write press releases and news stories to be sent to local media outlets for publication. The Communications Officer will also use online social networking sites, such as Facebook and Twitter to post program-related content. The use of these on-line formats will be a means to reach all beneficiaries within the MoE, including participating schools and others involved or interested in the LTD program. Furthermore, the LTD Communications Officer will maintain a close working relationship with public relation officers of the school districts to ensure that all parties are involved in reaching out to the largest audience possible.
