Community Resource Services. 13 4.11.1 City of Xxxxxxx (Xxxxxxx) shall provide Community Resource 14 Services to the following: parents and/or caregivers and their children ages 15 birth to eighteen (0-18) years who are at-risk of abuse or neglect. 16 Individuals may include: those who are or low-income or dealing with poverty 17 issues (hereinafter referred to as "PARTICIPANTS" for purposes of Subparagraph 18 4.11).

Community Resource Services. 21 4.11.1 IHA shall provide Community Resource Services (CRS) for low income or dealing with poverty issues ages birth to eighteen (0-18) years who are at risk of abuse and/or neglect; to the following: parents and/or caregivers and their children 22 individuals with children ages birth to eighteen (0-18) years who are at-risk, 23 or low-income 24 25 (hereinafter referred to as 26 "PARTICIPANTS" for purposes of Subparagraph 4.11). Community Resource Services. All Members will receive community services through our outreach program. The Masonic Charity Foundation of Connecticut supports this benefit. Your Wellness Coordinator will help to assist you in the comfort of your own home. This program is especially designed to help you with information and assistance related to Medicare, Assisted Living, Memory Care, and Skilled Nursing Home Care Eligibility for benefits, other insurance programs, planning for your future needs, etc.
Community Resource Services. 8 4.11.1 IHA shall provide Community Resource Services to the 9 following: parents and/or caregivers and their children ages birth to eighteen 10 (0-18) years who are at risk of abuse and/or neglect; low income or dealing 11 with poverty issues (hereinafter referred to as "PARTICIPANTS" for purposes of 12 Subparagraph 4.11).
Community Resource Services. 19 4.10.1 Raise Foundation (RF) shall provide Community Resource 20 Services (CRS) for at-risk, low-income to the following: parents and/or 21 caregivers and their children ages birth to eighteen (0-18) years who are at- 22 risk of abuse or neglect; or those who are low-income or dealing with poverty 23 issues hereinafter referred to as "PARTICIPANTS" for purposes of Subparagraph 24 4.10. Community Resource Services 25 4.10.2 RF shall provide CRS for a Community Resource Services 26 minimum of one thousand two hundred (1200) 27 PARTICIPANTS. CRS two thousand (2,000) unduplicated shall include, but not be 28 limited to the following: an assessment of need; referral to emergency 2 counseling and treatment; parenting training; utility assistance; health and 3 mental health treatment; education and job training; legal aid; youth academic 4 and recreation services; linkages to a wide range of community services; 5 general family support; family advocacy; case management team; parenting

Community Resource Services. 13 4.12.1 CAPOC shall provide Community Resource Services to the 14 following: parents and/or caregivers and their children ages birth to eighteen 15 (0-18) years who are at-risk of abuse or neglect; low-income or dealing with 16 poverty issues (hereinafter referred to as "PARTICIPANTS" for purposes of 17 Subparagraph 4.12).
