Content of Physician’s Statement. A physician's statement shall include: the beginning date and anticipated ending date of the illness; a statement that, in the physician’s opinion, the employee is unable to work; and, when applicable, any restrictions or limitations upon the employee’s return to work. The employee shall provide a copy of the physician’s statement, without the diagnosis, to the immediate supervisor. 7.1.5.5.1 When the time off falls under Section 7.6, Entitlement to Other Sick Leave or Section 7.22, Catastrophic Leave, the physician’s statement must include a diagnosis (medical condition) and shall be provided directly to the Employee Benefits Department to ensure confidentiality.

  • Annual Physical Examination A permanent employee shall be granted up to one day per year with pay for the purpose of a comprehensive physical examination provided that the verification of such an examination is submitted to the District.
