Conversion of Illness. Injury, and Emergency Leave Upon Retirement or Death Any employee who hereafter shall retire or who shall die while employed by the District may elect (personally or by his/her personal representative, as appropriate) to convert accumulated unused Illness, Injury, and Emergency Leave days to monetary compensation at the rate of twenty-five percent (25%) of the employee's full-time daily rate of compensation at the time of termination from employment for each full day of eligible Illness, Injury, and Emergency Leave to a maximum of 180 days. Any such conversion of Illness, Injury, and Emergency Leave upon retirement or death shall be subject to the terms and limitations of state statutes.

Conversion of Illness. Injury and Emergency Leave Upon Retirement/Separation from 11 Service or Death: Any employee who shall retire, separate from service or die while 12 employed by the District may elect (personally or by a personal representative, as 13 appropriate) to convert accumulated unused illness, injury and emergency leave days to 14 monetary compensation at the rate of 25 percent of the employee's full-time daily rate of 15 compensation at the time of retirement, separation from service or death for each full day 16 of eligible illness, injury and emergency leave up to a maximum of one hundred eighty 17 (180) days. Any such conversion of illness, injury and emergency leave upon retirement, 18 separation from service or death shall be in accordance with the law, including RCW 19 28A.400.210 and RCW 28A.400.212.

  • Notification of Illness Nurses should notify the Medical Center of absence from work because of illness as far in advance as possible, but at least three and one-half (3 ½) hours before the start of the nurse’s shift. Repeated failure to give such minimum notification will result in reduction of otherwise payable sick leave for that shift by two (2) hours. Repeated failure as used in this section means more than twice every two years.

  • Continuation of Insurance All policies of insurance shall provide for at least 30 days prior written cancellation notice to the Secured Party. In the event of failure by the Debtor to provide and maintain insurance as herein provided, the Secured Party may, at its option, provide such insurance and charge the amount thereof to the Debtor. The Debtor shall furnish the Secured Party with certificates of insurance and policies evidencing compliance with the foregoing insurance provision.

  • TERMINATION OF INSURANCE A. Your policy will lapse if you do not pay your premium when due.
