Common use of Data and Reporting Clause in Contracts

Data and Reporting. The Provider will submit, 1 (one) DCF-approved documentation form for each visit with child or youth, the invoice and the documentation notes by the 10th of the month following the month of service. The progress note shall include:  short and long term goals identified by the CPS team,  interventions,  strengths and areas for improvement,  relevance of the activity with regard to the child’s identified needs and goals,  any other information relevant to the service and required by the Department.
