Reporting Results a. All results are to be reported by SMS message to the Association Full-Time system by no later than 6pm (10pm for evening matches) on the day of the match. Failure to comply with this Rule will result in a fine in accordance with the Fines Tariff.
b. After the conclusion of each competition match, the Secretary of each club shall send to the Association within three days (Sundays excluded) the official form containing a list of players participating, the result of the match and a Club marking of the Referee. Failure to comply with this Rule will result in a fine in accordance with the Fines Tariff.
c. Should a Club intentionally submit a wrong or false name, such Club shall be charged with misconduct and be dealt with as the Association may determine.
d. The Association shall arrange the checking of team sheets and report to the Club or Clubs concerned any breaches discovered within four days of receipt of the team sheet.
Reporting Results. In addition to the reporting requirements as specified in 40 CFR part 792, the following infor- mation shall be reported.

Reporting Results. The results of a drug or alcohol test will be reported verbally and in writing to the Chief or the person he designates to receive those results. The testing laboratory will reveal to the designated official only whether the employee has received a result of negative for drugs or positive for drugs. The results of the drug test will be maintained in the strictest confidence by the Town and will not be disseminated except on a "need to know" basis.

Reporting Results a. The laboratory shall report tests results to the MRO within ten (10) business days after receipt of the specimen by the laboratory.
b. The laboratory shall report as negative to the MRO all specimens, which are negative on the initial, test or are negative on the confirmation test. Only specimens, which are confirmed as positive on the confirmation test, shall be reported positive to an MRO for a specific drug.
c. The laboratory shall transmit results to the MRO in a manner designed to ensure confidentiality of the information. The laboratory and MRO must ensure the security of the data transmission and restrict access to any data transmission, storage, and retrieval system.
d. The MRO and/or the tested employee may request from the laboratory, and the laboratory shall provide, quantitation of test results.
e. The MRO will also verify that positive and negative test results were properly analyzed and handled. The MRO will have knowledge of substance abuse disorders and shall also be knowledgeable in the medical use of prescription drugs and in the pharmacology and toxicology of illicit drugs. The MRO shall evaluate the drug test results which are reported by the laboratory, verify the drug test results by checking the chain of custody form that the specimen was collected, transported and analyzed under proper procedures as set forth in this policy.
f. The MRO will initially notify the employee or job applicant of a confirmed positive test result within five (5) business days of receipt of the test result from the laboratory, and determine if any alternate medical explanations caused a positive test result. This notification may be accomplished via telephone. This determination by the MRO shall include conducting a medical interview with the employee or job applicant, review of the employee’s or job applicant’s medical history, review of any other relevant bio-medical factors, a review of all medical records made available by the tested employee or job applicant, and an inquiry as to whether any prescription or non- prescription medications could have caused the positive test result. The MRO will provide an opportunity for the employee or job applicant to discuss the positive test result and to submit documentation of any prescriptions relevant to the positive test result for up to five (5) business days after notification period.

Reporting Results. The results of supplemental identification and evaluation efforts, if such efforts are required, will be submitted to the appropriate SHPO for review. DoD will submit the results to SHPO within three months of completion of supplemental work.
a. In conducting supplemental identification and evaluation efforts, DoD will ensure that documentation consistent with 36 CFR §800.11 is submitted to the appropriate SHPO. The level of effort for identification of historic properties will be consistent with 36 CFR §800.4(b)(1) and published SHPO survey guidelines, if any, where these do not conflict with applicable federal guidelines.
b. SHPO will comment on results provided by DoD within 45 calendar days of the date the documentation is received by the SHPO. If SHPO agrees with the results, DoD may proceed accordingly. If DoD has received no written comments from the SHPO within 45 days after submission of the results to the SHPO, DoD may assume agreement and proceed accordingly.
c. If SHPO disagrees with the results, SHPO will provide written notification to DoD within 45 days from the date SHPO received the results. The notice should include the reasons for the disagreement and SHPO’s proposed resolution. DoD will take SHPO’s comments into account and advise SHPO of DoD’s response. If the parties still disagree, DoD and SHPO will consult for an additional period not to exceed 15 days from the date of DoD’s notice of its proposed response. If consensus cannot be reached within 15 days, DoD will provide written notice to the Signatories, Invited Signatories, and Concurring Parties of DoD’s decision, and further disagreement, if any, shall be addressed in accordance with Stipulation XIII.B. The party referring the matter for resolution under Stipulation XIII.B must provide DoD with written notification of the referral within 10 days of DoD’s notice of decision.

Reporting Results. The laboratory will report all test results. As to urine tests, the laboratory will report as positive only those specimens confirmed positive by GC/MS. The laboratory will report the results only in writing and will report only to the Assistant Superintendent. The laboratory will transmit to the Assistant Superintendent a copy of the Toxicology Chain of Custody form and Consent form for all positive test results. The District shall provide copies of such documents to the employee.
