DATA TO PROVIDER. Alliance shall provide data to the Provider related to delivery of Services under this Contract such as:
a. Performance feedback reports or information to the provider, if compensation is related to efficiency criteria;
b. Information on benefit exclusions, where applicable;
c. Administrative and utilization management requirements;
d. Credential verification programs;
e. Quality assessment programs; and
f. Provider sanction policies. Notification of changes in these requirements shall also be provided by Alliance on the Alliance website, in advance of the effective date of any changes in order to allow Providers time to comply with such changes.
DATA TO PROVIDER. United agrees to provide data and information to the Provider as required by the State Contract, including:
i) Performance feedback reports or information to the Provider, if compensation is related to efficiency criteria;
ii) Information on benefit exclusions; administrative and utilization management requirements; credential verification programs; quality assessment programs; and provider sanction policies;
iii) Notification of changes in these requirements shall also be provided by the United, allowing Provider time to comply with these changes.
DATA TO PROVIDER. Carolina Complete will provide certain data and information to the Provider, and changes to such information, which may include performance feedback report if compensation is related to efficiency criteria, information on benefit exclusions, administrative and utilization management requirements; credential verification programs; quality assessment programs; and provider sanction policies (Section VII, G(1)(n)).
DATA TO PROVIDER. Sandhills Center shall furnish to Provider any such data and information as may be necessary for Provider to fulfill its obligations to Sandhills Center under this Agreement. Such data and information includes, but is not necessarily limited to: (a) performance feedback reports or information if compensation is related to efficiency criteria; (b) information on benefit exclusions, administrative and utilization management requirements, credential verification programs, quality assessment programs, and provider sanction policies; and (c) notification of changes in these requirements. Plan may provide such information through its Provider Manual and periodic updates thereto.
