Signature Date INTENDED USE STATEMENT Access to the DVS record information system is granted under Title 18 U.S.C. § 2721. Subd. (b) (1) which states: For use by any government agency, including court or law enforcement agency, in carrying out its functions or any private person or entity acting on behalf of a Federal, State or local agency in carrying out its functions. Motor vehicle and driver license agents perform a number of functions related to DVS transactions. They accept motor vehicle and driver license applications, verify identification, screen vision, and take photos. An important part of their function is to collect fees and taxes. They are accountable for documenting their transactions and properly transmitting documents and information to DVS. I, the undersigned, as the appointee, certify by signing this document the information and statements provided on this document are true and correct and I agree to the terms and conditions for the intended use of DVS data as defined in Exhibit B and Exhibit C, which are attached and incorporated into this agreement by reference. I understand that pursuant to Minn.Stat. § 171.12, Subd. 1a (b), the Commissioner will immediately and permanently revoke access of any staff who willfully enters, updates, accesses, shares, or disseminates data in violation of state or federal law. However, neither Party may assign any of its rights or delegate any of its obligations under this Agreement without the prior written consent of the other Party, which consent shall not be unreasonably withheld or delayed.
