Dental Services Plan. The Employer agrees to pay 100% of the monthly premiums, at a dependant rate if required, for all regular full-time employees entitled to coverage under the dental plan, and remit same premiums to the Union.
Dental Services Plan. The Corporation agrees to provide a Dental Plan for the benefit of Regular Full-Time Employees who have completed six (6) months of continuous service and Temporary Full-Time Employees who have completed twelve (12) months of continuous service which provides for the following services:
(a) Basic Dental Services (Plan A) paying for eighty percent (80%) of the approved schedule of fees (to a maximum of $1250.00 per person per calendar year).
Dental Services Plan. A Regular Employee who has completed six (6) months of continuous service shall be covered under the Dental Services Plan, which provides the following services subject to the terms and conditions of the Plan:
(1) Basic Dental Services (Plan A) paying eighty percent (80%) of the approved schedule of fees;
(2) Prosthetics, Crowns and Bridges (Plan B) paying for fifty percent (50%) of the approved schedule of fees;
(3) Orthodontics (Plan C) paying for fifty percent (50%) of the approved schedule of fees to a lifetime maximum of $3000.00 per person covered for adults and dependent children as defined in the Plan.
Dental Services Plan. Regular Full-Time Employees who have completed the probationary period of six (6) months, as described in Article 2, and Temporary Full-Time Employees who have completed such probationary period and an additional six (6) months' service, shall be entitled to be enrolled in the Dental Services Plan and to be covered effective the first day of the calendar month following completion of the required period of service. A spouse, dependent children to age 21, and dependent children over age 21 in full-time attendance at a recognized school or college shall be covered as employee dependents. Dental services will be covered in accordance with the Plan as follows: Plan “A” – 90% diagnosis, prevention, surgery, restorations, denture repairs, endodontics, gum treatment and, effective 2017 January 01, white fillings Plan “B” – 60% crowns, bridges and partial or complete dentures Plan “C” – 50% orthodontic coverage, fifty percent (50%) to a maximum benefit of $4500 per person per lifetime (adults and children) with immediate coverage on the effective date and no run-off on termination of the Plan The City shall pay one hundred percent (100%) of the premiums required for the Plan.
Dental Services Plan. The Employer agrees to provide a dental plan for the benefit of employees, effective the first of the month following date of hire, which provides for the following services:
(a) Basic Dental Services (Plan A) paying for 80% of the approved schedule of fees;
(b) Prosthetics, Crowns and Bridges (Plan B) paying for 50% of the approved schedule of fees;
(c) Orthodontics (Plan C) paying for 50% of the approved schedule of fees to a lifetime maximum of $3000 for dependent children and adults as defined by the Plan;
(d) The Employer shall pay seventy-five percent (75%) of the premium and the employees shall pay twenty-five percent (25%) of the premium.
Dental Services Plan. The Employer and the Union agree that the following dental plan has been in effect for the benefit of those employees participating therein under the terms of this Collective Agreement since 1 August 1974 and that it shall continue as amended hereafter:
(a) Basic Dental Services (Plan A--provision for payment of 100% of the approved schedule of fees);
(b) Prosthetics (Plan B--provision for payment of 70% of the approved schedule of fees); and
(c) Orthodontics (Plan C - provision for payment of seventy percent (70%) of the approved schedule of fees). The lifetime maximum shall be $3000 for employees and their dependents as defined by the Plan. The cost of premiums shall be borne 100% by the Employer. All employees shall participate in the dental plan except those employees who were exempted from participation when the dental plan was first established. Any employee who was exempted from participation in the dental plan as aforesaid shall not be eligible for participation therein.

(d) The City shall pay one hundred percent (100%) of the premiums required for the plan.
Dental Services Plan. The Employer and the Union agree that members shall be entitled to the following dental plan:
(a) Basic Dental Services (Plan A—provision for payment of 85% of the approved schedule of fees);
(b) Prosthetics (Plan B—provision for payment of 70% of the approved schedule of fees); and
(c) Orthodontics (Plan C—provision for payment of 70% of the approved schedule of fees). The lifetime maximum shall be $3000.00 for employees and their dependents as defined by the Plan. All members shall participate in the dental plan except those members who were exempted from participation when the dental plan was first established. Any member who was exempted from participation in the dental plan as aforesaid shall not be eligible for participation therein. The cost of premiums shall be borne 100% by the Employer.
Dental Services Plan. The Employer has established a dental plan for all Regular Full-Time Employees, effective the first day of the month following the date of hire, on the following basis:
(a) Basic Dental Services (Plan A) paying for eighty percent (80%) of the approved schedule of fees;
(b) Prosthetics, Crowns and Bridges (Plan B) paying for fifty percent (50%) of the approved schedule of fees;
(c) Orthodontics (Plan C) paying for fifty percent (50%) of the approved schedule of fees. The lifetime maximum shall be three thousand dollars ($3,000) for adults and dependent children as defined by the Plan.
(d) The Employer shall pay eighty-five percent (85%) and the employees shall pay fifteen percent (15%) of the premiums.
Dental Services Plan. The regular employees who work at least 20 hours per week are entitled to be covered by the Union's Dental Services Plan effective upon completion of the third month of employment. The University shall pay 100% of the monthly premium for such employees, at the dependent rate, if required.
