Accordingly, he does not treat any specific health conditions. As a Holistic Health Practitioner and Traditional Naturopath he can assist me by educating me about your body and its proper nutritional support using whole foods, herbs, nutritional supplements, and other natural modalities. Xx. Xxxxxxx’x role is to educate me about healthy living, eating habits, nutritional supplements, and other natural modalities that have been beneficial for other people with similar circumstances. I understand that Xx. Xxxxxxx cannot cure or heal anything and therefore does not guarantee any beneficial results. The body is a complex organism and there are many factors that are involved in obtaining improved health. I understand that only my own body has the healing power to improve my health and that by my increased education and awareness, along with helpful nutritional support, I may help the body to initiate its own healing processes. I understand that only my own actions in supporting my body’s own healing power can determine whether my health is improved or not. I also understand that stress and various emotional or psychological conditions can affect my physical health. I agree to inform Xx. Xxxxxxx of any drugs or supplements that I am taking so that he can help minimize any potential interactions.

Subject to DHCS provider enrollment certification requirements, the Contractor shall maintain continuous availability and accessibility of covered services and facilities, service sites, and personnel to provide the covered services through use of DMC certified providers.
