Doctor’s Certification or Other Proof. At the discretion of the employee’s supervisor, a physician’s certificate may by required for any period of absence for which sick leave is claimed; however, when absence is for more than five (5) consecutive workdays the employee shall file a physician’s certificate with the Division Manager stating the cause of the absence. The physician’s certificate shall be applicable for every day of the employee’s absence.
