Documentary Evidence. (a) To be entitled to payment for sick leave the employee, when required by the Company, must provide a medical certificate from a registered health practitioner that states that in the registered health practitioner’s opinion: (i) the employee is, was, will be unfit for work during the period because of personal illness or injury or; (ii) the employee’s immediate family member had, has, or will have a personal illness or injury during the period. (b) If the employee cannot obtain a medical certificate, a statutory declaration may be considered acceptable in certain circumstances.
