Common use of Dues and Initiation Fees Clause in Contracts

Dues and Initiation Fees. The Employer shall deduct Initiation Fees and the weekly regular union dues and special assessments in the amount and manner specified by the Union By Laws and Constitutions from each pay cheque due to each employee covered by this Agreement and remit such monies so deducted to the United Food and Commercial Workers Canada Local 175 presently located at 0000 Xxxxxxxx Xxxx, Xxxxxxxxxxx, Xxxxxxx, X0X 0X0, on or before the 15th day of the month following the month in which such deductions are made. The Employer will at the same time submit a remittance statement of the employees from whose pay such deductions have been made. In the event that the amount of such deduction are changed during the term of this Agreement, the Employer shall be given fourteen (14) days’ prior notice. The remittance statement shall be documented by location containing a dues and initiation report which will be provided in the form of e-mail (xxxxx@xxxx000.xxx) or on a computer diskette as well as a hard copy of the dues report being attached to the remittance cheque. The information provided shall be on a standard spreadsheet in Excel, Quattro Pro, Lotus or other software program acceptable and adaptable to the Union. The spreadsheet will be in a format provided by the Union and the Employer will provide the following information: as known to the Employer. 1. Social Insurance Number 2. Employee number, if applicable 3. Full name (Last/First/Initials) 4. Full address, including City and Postal Code 5. Telephone number (including area code)
