Eligibility; Card Activation and Replacement. You must have an Account in good standing to receive a Card. We may refuse to issue Cards to you in our sole discretion. To activate your Card, log in to your Account and follow the Card activation instructions, or follow the instructions you received with your Card. Please sign the signature panel on the back of the Card as soon as you receive it (if applicable). When you activate your Card, you must select a personal identification number (“PIN”) to withdraw cash at automated teller machines (“ATM”) and for certain Transactions at merchant locations where a PIN is required. Keep your PIN a secret. If your PIN or Card is lost or stolen, log in to your Account to block your Card, or notify us by emailing us at xxxx@xxxxxxx.xxx. If you need to replace your Card, please log into your Account to request a replacement Card.

  • Card Replacement If you need to replace the Card for any reason except at the time of Card expiration, please contact (000) 000-0000 to request a replacement. You will be required to provide personal information which may include your Account number, 16-digit Card number, full name, transaction history, copies of acceptable documentation, etc. Standard shipping of your replacement card will be provided without a fee. Expedited shipping is available via FedEx; however, an Expedited Shipping Fee of $50 will be assessed.

  • SERVICE AND REPLACEMENT PARTS 23.1 During the term of this Contract, Seller will fulfill Buyer's and Customer’s service and replacement parts requirements at the then current production price(s) under this Contract. If the Products are systems or modules, Seller will sell the components or parts that comprise the system or module at price(s) that will not, in the aggregate, exceed the price of the system or module less assembly costs.

  • Orientation and In-Service Program The Hospital recognizes the need for a Hospital Orientation Program of such duration as it may deem appropriate taking into consideration the needs of the Hospital and the nurses involved.

  • Replacement of Key Personnel The Engineer must notify the State in writing as soon as possible, but no later than three business days after a project manager or other key personnel is removed from association with this contract, giving the reason for removal.

  • State Approval of Replacement Personnel The Engineer may not replace the project manager or key personnel without prior consent of the State. The State must be satisfied that the new project manager or other key personnel is qualified to provide the authorized services. If the State determines that the new project manager or key personnel is not acceptable, the Engineer may not use that person in that capacity and shall replace him or her with one satisfactory to the State within forty-five (45) days.

  • Outpatient emergency and urgicenter services within the service area The emergency room copay applies to all outpatient emergency visits that do not result in hospital admission within twenty-four (24) hours. The urgicenter copay is the same as the primary care clinic office visit copay.
