Employee Qualifications. The employee confirms that he/she meets the minimum qualifications for employment as required by the Self-Direction Program and described in the Self-Direction Program regulations (8.314.6 NMAC or 8.308.12 NMAC), the Self-Direction Program Service Standards, and the Centennial Care Managed Care Policy Manual.
