Common use of Establishment of Service Clause in Contracts

Establishment of Service. 6.1 After receiving certification as a local exchange carrier from the applicable regulatory agency, Quality Telephone will provide the appropriate BellSouth Advisory team manager the necessary documentation to enable BellSouth to establish accounts for resold services ("master account"). Quality Telephone is required to provide the following before a master account is established: blanket letter of authorization, misdirected number form, proof of PSC/PUC certification, the Application for Master Account, an Operating Company Number ("OCN") assigned by the National Exchange Carriers Association ("NECA") and a deposit and tax exemption certificate, if applicable. 6.1.1 If Quality Telephone needs to change its OCN(s) under which it operates when Quality Telephone has already bee conducting business utilizing those OCN(s), Quality Telephone shall bear all costs incurred by BellSouth to convert Quality Telephone Quality Telephone to the new OCN(s). OCN conversion charges include all time required to make system updates to all of Quality Telephone's end user customer records. Appropriate charges will appear in the OC&C section of Quality Telephone's xxxx. 6.2 Quality Telephone shall provide to BellSouth a blanket letter of authorization ("LOA") certifying that Quality Telephone will have End User authorization prior to viewing the End User's customer service record or switching the End User's service. BellSouth will not require End User confirmation prior to establishing service for Quality Telephone’s End User customer. 6.3 BellSouth will accept a request directly from the End User for conversion of the End User's service from Quality Telephone to BellSouth or will accept a request from another CLEC for conversion of the End User's service from Quality Telephone to such other CLEC. Upon completion of the conversion BellSouth will notify Quality Telephone that such conversion has been completed.

Quality Telephone C & M Total Communications is required to provide the following before a master account is established: blanket letter of authorization, misdirected number form, proof of PSC/PUC certification, the Application for Master Account, an Operating Company Number ("OCN") assigned by the National Exchange Carriers Association ("NECA") and a deposit and tax exemption certificate, if applicable. 6.1.1 If Quality Telephone C & M Total Communications needs to change its OCN(s) under which it operates when Quality Telephone C & M Total Communications has already bee conducting business utilizing those OCN(s), Quality Telephone C & M Total Communications shall bear all costs incurred by BellSouth to convert Quality Telephone Quality Telephone C & M Total Communications C & M Total Communications to the new OCN(s). OCN conversion charges include all time required to make system updates to all of Quality TelephoneC & M Total Communications's end user customer records. Appropriate charges will appear in the OC&C section of Quality TelephoneC & M Total Communications's xxxx. 6.2 Quality Telephone C & M Total Communications shall provide to BellSouth a blanket letter of authorization ("LOA") certifying that Quality Telephone C & M Total Communications will have End User authorization prior to viewing the End User's customer service record or switching the End User's service. BellSouth will not require End User confirmation prior to establishing service for Quality TelephoneC & M Total Communications’s End User customer. 6.3 BellSouth will accept a request directly from the End User for conversion of the End User's service from Quality Telephone C & M Total Communications to BellSouth or will accept a request from another CLEC for conversion of the End User's service from Quality Telephone C & M Total Communications to such other CLEC. Upon completion of the conversion BellSouth will notify Quality Telephone C & M Total Communications that such conversion has been completed.
