Following all trainings, participants will be requested to fill out a course evaluation form to assist the program coordinators with assessing the effectiveness of the instructor, materials and information offered in the trainings. Changes will be made if a minimum of 75% of the class does not indicate an average score of 3.0 (“good”) or better on a five point scale, with 5.0 being “excellent”. If fewer than 75% of the class rates the quality of the course as 3.0 or better, the instructor and curriculum shall be reviewed and steps taken to assure success in subsequent trainings.
