Form 1295. The Developer will provide a completed and notarized Form 1295 generated by the Texas Ethics Commission's electronic filing application in accordance with the provisions of Section 2252.908 of the Texas Government Code and the rules promulgated by the Texas Ethics Commission (a "Form 1295"), in connection with entry into this Agreement. Upon receipt of the Developer's Form 1295, the City agrees to acknowledge the Developer's Form 1295 through its electronic filing application. The Developer and the City understand and agree that, with the exception of information identifying the City and the contract identification number, the City is not responsible for the information contained in the Developer's Form 1295 and the City has not verified such information.

Form 1295. Submitted herewith is a completed Form 1295 in connection with the Developer’s participation in the execution of this Agreement generated by the Texas Ethics Commission’s (the “TEC”) electronic filing application in accordance with the provisions of Section 2252.908 of the Texas Government Code and the rules promulgated by the TEC (the “Form 1295”). Form 1295. Developer acknowledges that Section 2252.908, Texas Government Code (“Section 2252.908”) requires disclosure of certain matters by business entities entering into a contract with a local government entity such as the City. Developer confirms that it has reviewed Section 2252.908 and that Developer will 1) complete Form 1295, using the unique identification number specified on page 1 of this Amendment, and electronically file it with the Texas Ethics Commission (“TEC”); and 2) submit to the City the signed and notarized Form 1295, including the certification of filing number of the Form 1295 with the TEC, at the time the Developer executes and submits this Amendment to the City.
