Form of Billing. The Town’s monthly billing shall consist of the sum of the (1) BOD Surcharge Cost, (2) TSS Surcharge Cost, and (3) Industrial User Surcharge Cost as provided in the Town’s Policy Regarding Surcharge Assessments for Discharge of High-Strength Waste to the Town’s Sewer System; and (4) a Flow Component Cost as follows: - January 1, 2019 to September 30, 2019: $5.52 per 1,000 gallons of wastewater; - October 1, 2019 to December 31, 2019: $5.68 per 1,000 gallons of wastewater; - January 1, 2020 to December 31, 2020: $5.91 per 1,000 gallons of wastewater; and - January 1, 2021 to December 31, 2021: $6.14 per 1,000 gallons of wastewater. Domestic wastewater discharged will be charged at the rate charged all other sewer users, currently $6.78/1,000 gallons of domestic discharge.

Domestic wastewater discharged will be charged at the rate charged all other sewer users, currently $6.94/1,000 gallons of domestic discharge.

  • Form of Application The Contractor shall periodically submit to the Design Professional an Application for Payment on the form set forth in Section 7 (sometimes called a "Periodical Estimate") for each payment requested, and, if requested by the Owner or Design Professional, shall attach backup materials including, but not limited to, receipts or other vouchers, showing his payments for materials and labor, including payments previously made to Subcontractors.

  • Form of Order 7.1 Subject to paragraphs 1 to 6 above, each Contracting Body may place an Order with the Supplier by serving an order in writing in substantially the form set out in Framework Agreement Schedule 4 (Order Form & Call-Off Terms).   • Form of Agreement If a vendor submitting an Proposal requires TIPS and/or TIPS Member to sign an additional agreement, a copy of the proposed agreement must be included with the proposal. In response to submitted supplemental Vendor Agreement documents, TIPS will review proposed vendor Agreement documents. Supplemental Vendor’s Agreement documents shall not become part of TIPS’s Agreement with vendor unless and until an authorized representative of TIPS reviews and approves it.

  • Form of Contract The form of contract for this solicitation shall be the Request for Proposal, the awarded proposal(s) and best and final offer(s), and properly issued and reviewed purchase orders referencing the requirements of the Request for Proposals. If a vendor submitting an offer requires TIPS and/or TIPS Member to sign an additional agreement, a copy of the proposed agreement must be included with the proposal. Vendor contract documents: TIPS will review proposed vendor contract documents. Vendor’s contract document shall not become part of TIPS’s contract with vendor unless and until an authorized representative of TIPS reviews and approves it.

  • FORM OF TENDER 2.8.1 The tenderers shall complete the Form of Tender and the appropriate Price Schedule furnished in the tender documents, indicating the services to be performed.

  • Form of Bonds The Bonds, the form of Trustee’s certificate of authentication, and the form of assignment to appear thereon, shall be substantially in the respective forms set forth in Exhibit A attached hereto and by this reference incorporated herein, with necessary or appropriate variations, omissions and insertions, as permitted or required by this Indenture.

  • Account Description This account is a term share account with a maturity of two (2) years or four (4) years in which the member generally does not have the right to make withdrawals for six (6) days after the account is opened unless an early withdrawal penalty is imposed.
