Common use of Identification Card Clause in Contracts

Identification Card. (A) The Contractor shall issue an identification card to all Enrollees. The identification card shall contain the following information: (1) Children’s Health Insurance Program (CHIP); (2) Whether the Enrollee is, Plan B or Plan C, and if they are American Indian/Alaska Native. The Contractor will begin including the term American Indian/Alaska Native within 120 days of the start of the contract begin date. (3) The name of Contractor’s Dental Plan; (4) A toll free Member Service number. (B) The Contractor shall issue the identification card to new Enrollees within 21 calendar days after the Department notifies the Contractor that the Enrollee has been enrolled in the Contractor’s Dental Plan. (C) The Dental Plan must issue a new identification card to incumbent Enrollees when their coverage terminates for 60 days or more before reinstating with the same Dental Plan, when the Enrollee changes Plans (B or C), or when the Enrollee has changes to their co-pay exempt status. (D) The Dental Plan is not required to issue an identification card to Enrollees who have qualified for only retroactive enrollment in the Contractor’s Dental Plan.
