Individual Support and Service Plan. A written plan developed by an IRIS participant and their legal decision maker (if applicable) that lists the goods, supports, and services chosen by the participant to meet their long-term care needs and outcomes; the cost of services; their frequency; and the provider of each service. Unpaid goods and support, as well as Medicaid-funded services received, are also listed on the plan, as well as goods, supports, and services received at no cost.
