Infrastructure Payments. Effective through December 31, 2013, a portion of the $30 million total computable demonstration funds may be allocated to support health care delivery system infrastructure costs for ConnectCare only. The infrastructure payments are to support activities/initiatives that will be used to improve access, improve provider operations, and increase business efficiencies. These payments shall not be duplicative of other cost considerations in the service payment rates to ConnectCare providers or any other payment provided under the demonstration. Activities may include, but are not limited to, costs associated with purchasing billing software, hardware for systems, and/or costs associated with extended hours of operations.

Infrastructure Payments. (a) If the ACO selects and CMS approves participation in Infrastructure Payments, CMS shall make payments to the ACO in accordance with the methodology in Appendix G.
