Kaiser Dental Plan. 8 Increase office visit co-pay from five dollars ($5) to ten dollars 9 ($10). 10 Orthodontia (Plan U) for children under 18 only: Maximum Plan 11 benefit payment three-thousand dollars ($3,000) per child. Member pays fifty 12 percent (50%) of charges up to the maximum Plan payment and one-hundred 13 percent (100%) of charges in excess of the maximum Plan payment. (Any 14 covered dependent who has been banded prior to January 1, 2010 under the 15 existing Kaiser Orthodontic program will remain eligible to receive the existing 16 orthodontic benefit (three-hundred dollar ($300) maximum member co-pay) for 17 remainder of current orthodontic treatment plan or treatment phase.
