Licenses, Certifications, Credentialing and Privileging Requirements a. The PROVIDER shall obtain and maintain during the term of this Agreement all licenses, certifications, registrations, National PROVIDER Identifier pursuant to Section 5005 and section 12006 of the 21st Century Cures Act, accreditations, authorizations, and approvals required by federal, State, and local laws, ordinances, rules and regulations for the PROVIDER to operate and provide Medicaid and/or non-Medicaid programs and services within the State of Michigan.

Licenses, Certifications, Credentialing and Privileging Requirements. The PROVIDER shall obtain and maintain during the term of this Agreement all licenses, certifications, registrations, National Provider Identifier pursuant to Section 5005 and section 12006 of the 21st Century Cures Act, accreditations, authorizations, and approvals required by federal, State, and local laws, ordinances, rules and regulations for the PROVIDER to operate and provide Medicaid and/or non-Medicaid programs and services within the State of Michigan. The PROVIDER shall insure that there are systems in place to assure that its staff meet appropriate licensure, competency, and criminal history standards including systems for the primary verification of professional credentials. PROVIDERs must perform criminal background checks on their employees, including central registry checks for child care staff working with minor children. These criminal background checks are a requirement of this Agreement. PROVIDER must have, and follow, a policy on hiring of persons with criminal backgrounds that is consistent with 333.18263 of the Public Health Code, Act 368 of 1978 for Behavior Technicians, the Social Security Act (42 USC 1320a-7(a), the Michigan Medicaid Manual, and applicable licensing and/or certification rules. Criminal background checks (CBC) shall be conducted prior to any consumer contact, generally no later than the first few days of hire. An offer of employment may be made contingent on a CBC and should expressly note that there is to be no contracted service provided until all criminal background checks are complete. CBC, including central registry checks shall be repeated at a frequency defined in CMHSP Participant policy for all persons who have direct contact with consumers

  • Credentialing Requirements Registry Operator, through the facilitation of the CZDA Provider, will request each user to provide it with information sufficient to correctly identify and locate the user. Such user information will include, without limitation, company name, contact name, address, telephone number, facsimile number, email address and IP address.

  • Credentialing The Provider will maintain written documentation confirming that each individual providing services under this agreement has and maintains the requisite credentials. Any change in status regarding any credentialing requirements must be reported in writing, by the Provider to the Department's Credentialing Contracted Agent, within thirty days.

  • Provider Credentialing Contractor shall perform, or may delegate activities related to, credentialing and re-credentialing Participating Providers in accordance with a process reviewed and approved by State Regulators.

  • Minimum Site Requirements for TIPS Sales (when applicable to TIPS Sale). Cleanup: When performing work on site at a TIPS Member’s property, Vendor shall clean up and remove all debris and rubbish resulting from their work as required or directed by the TIPS Member or as agreed by the parties. Upon completion of work, the premises shall be left in good repair and an orderly, neat, clean and unobstructed condition. Preparation: Vendor shall not begin a project for which a TIPS Member has not prepared the site, unless Vendor does the preparation work at no cost, or until TIPS Member includes the cost of site preparation in the TIPS Sale Site preparation includes, but is not limited to: moving furniture, installing wiring for networks or power, and similar pre‐installation requirements. Registered Sex Offender Restrictions: For work to be performed at schools, Vendor agrees that no employee of Vendor or a subcontractor who has been adjudicated to be a registered sex offender will perform work at any time when students are, or reasonably expected to be, present unless otherwise agreed by the TIPS Member. Vendor agrees that a violation of this condition shall be considered a material breach and may result in the cancellation of the TIPS Sale at the TIPS Member’s discretion. Vendor must identify any additional costs associated with compliance of this term. If no costs are specified, compliance with this term will be provided at no additional charge. Safety Measures: Vendor shall take all reasonable precautions for the safety of employees on the worksite, and shall erect and properly maintain all necessary safeguards for protection of workers and the public. Vendor shall post warning signs against all hazards created by the operation and work in progress. Proper precautions shall be taken pursuant to state law and standard practices to protect workers, general public and existing structures from injury or damage. Smoking: Persons working under Agreement shall adhere to the TIPS Member’s or local smoking statutes, codes, ordinances, and policies.

  • Academic Requirements A. The Institutions agree to follow the joint program curriculum and course by course articulation delineated in Appendix 1, which is attached hereto and made a part of this Agreement.

  • Child Abuse Reporting Requirements A. Grantees shall comply with child abuse and neglect reporting requirements in Texas Family Code Chapter 261. This section is in addition to and does not supersede any other legal obligation of the Grantee to report child abuse.

  • CERTIFICATION REGARDING DRUG-FREE WORKPLACE REQUIREMENTS 1. The Contractor certifies that it will provide a drug-free workplace by:

LEA shall ensure that its annual notice under FERPA includes vendors, such as the Provider, as “School Officials.”
