Common use of Mail Order Prescription Drugs Clause in Contracts

Mail Order Prescription Drugs. Subject to the terms and limitations set forth in this rider, Medically Necessary outpatient Prescription drugs are covered when dispensed by the Participating Mail Order Pharmacy designated by HMO and when prescribed by a Provider licensed to prescribe federal legend prescription drugs. Members are required to obtain prescriptions greater than a 30 day supply from the designated Participating Mail Order Pharmacy. Outpatient prescription drugs will not be covered if dispensed by a Participating Mail Order Pharmacy in quantities that are less than a 31 day supply or more than a 90 day supply (if the Provider prescribes such amounts).
