MEDICAL CARE CONSENT. If the Participant becomes injured, I authorize CYC to consent on my behalf to medical or dental care or both, including any X-ray examination, anesthetic, medical, dental or surgical diagnosis or treatment or hospital care under the general or special supervision and upon the advice of or to be rendered by a licensed physician or dentist, without liability to CYC.
MEDICAL CARE CONSENT. Participant authorizes Boosters to provide Participant with first aid or to obtain medical assistance, including transportation and emergency medical services. It is understood that the cost of such medical assistance is at Participant’s expense. This consent does not impose a duty upon the Boosters to provide such medical assistance.
MEDICAL CARE CONSENT. In the event that a medical situation arises and we are unable to contact you using the information you have provided, the doctor(s) at Xxxxxxxxx Animal Hospital are authorized to treat my pet at their discretion and at my expense. Minor Medical Care
MEDICAL CARE CONSENT. Parent authorizes Client to provide to Student first aid and, through medical personnel of Client’s choice, medical assistance, transportation, and emergency medical services.
