Non-Surgical Periodontics. We cover periodontal maintenance following documented periodontal surgery up to two (2) times per plan year if at least three (3) months have passed since the completion of active periodontal surgery. We cover periodontal scaling and root planing once (1) per thirty-six (36) month period, per quadrant. Predetermination is recommended for this service.
