Common use of Physician's Statement Clause in Contracts

Physician's Statement. If medical attention is required, the employee shall be required to furnish a statement from a licensed physician notifying the Employer that the employee was unable to perform his/her duties. Where sick leave is requested to care for a member of the immediate family, the Employer may require a physician’s certificate to the effect that the presence of the employee is necessary to care for the ill person.
