Common use of Planning Group Clause in Contracts

Planning Group. The Grantee shall convene an Advisory Group for ongoing operations and expansion of CHT(s), which shall be representative of local community health and human services organizations and stakeholders. Invitees to these groups should include, but are not limited to: • All willing area primary care practices, including Grantee-owned practices, community/independent practices that are not owned by the Grantee, Blueprint- recognized practices, and practices that are not recognized by the Blueprint • Hospital administrators and staff • Clinical and IT leadership • Medical and non-medical providers from community service organizations • The area-designated mental health and substance abuse agencies and area mental health and substance abuse providers • Public health leadership from Vermont Department of Health (VDH) local district offices • Agency of Human Services (AHS) field services director and leaders of local AHS initiatives, such as: o Children’s Integrated Services (CIS) o Enhanced Family Services (EFS) o Adult Local Interagency Team (LIT) • Consumer/patient representative(s) • Vermont Chronic Care Initiative coordinator(s) • Designated Regional Housing Organization (DRHO) leaders • Support and Services at Home (SASH) staff In consultation with the advisory group, the Grantee shall continue to update the CHT Plan and provide bi-annual updates in writing to the State’s Blueprint Associate Director. Upon approval of updates to the CHT Plan by the State’s Blueprint Associate Director, the Grantee will continue to either directly hire or subcontract for the CHT members based on the approved revisions to the CHT Plan.
