Probation Performance Report. A performance report for employees will be prepared by the immediate supervisor prior to the conclusion of the probation period per Section 5.0 or any extension thereof and annually thereafter. Each performance evaluation must be conducted by the employee’s immediate supervisor. Each report must be reviewed by the evaluator’s supervisor prior to discussion with the employee.
