Procedure for exclusion. You may exclude yourself from the Settlement by mailing to [ADMINISTRATOR], located at [ADDRESS], on or before [DATE], a written statement expressing your desire to be excluded from the Settlement in the ▇▇▇▇▇▇▇ litigation. If you wish to request exclusion from the Settlement in this Action, your written statement must include your name (and former names, if any), current address, telephone number, last four digits of your social security number, and the location and dates of your employment by Defendants. In addition, it must be postmarked on or before [DATE]. Requests for exclusion that do not include all required information, or that are not submitted on a timely basis, will be deemed null, void and ineffective. Persons who are eligible to and do submit valid and timely requests for exclusion from the Settlement will not receive Individual Settlement Awards, nor will they be bound by the terms of the proposed Settlement, if it is approved, or the Final Judgment in this Action.
Procedure for exclusion. 1. The decision of the Council can only be taken upon expiration of the period given to the faulty Part to meet its obligations.
2. If the Council wishes to exclude a Party from the European Corridor Management Agreement, the last notice of default must explicitly state that:
a. this notice of default may lead to the exclusion of the Party from the European Corridor Management Agreement;
b. the faulty Party will have a final opportunity to meet its obligations and remedy its fault within 30 calendar days starting from the date the last notice of default was sent;
c. the faulty Party shall be given a one-off opportunity to be heard.
