Procedure for Requesting Leaves. Requests for a leave of absence must be submitted in writing by the employee to the Prosecutor at least twenty- eight (28) days in advance of the date the leave is to commence, except in emergency situations. The request for the leave of absence shall state the reason for the leave and the exact dates on which the leave is to begin and end. Authorization or denial of a leave of absence shall be furnished to the employee in writing by the Prosecutor within fourteen (14) calendar days after the request for a leave of absence is received by the Prosecutor. Any request for an extension of a leave of absence must be submitted in writing to the Prosecutor at least fourteen (14) days in advance of the expiration date of the original leave, stating the reasons for the extension request and the exact revised date the employee is expected to return to work. Authorization or denial of the extension request shall be furnished in writing to the employee by the Prosecutor.
