Procedure for Requesting Unpaid Personal Leave. Requests for an unpaid personal leave of absence must be submitted in writing by the employee to the Elected Official or Department Head at least thirty (30) days in advance of the date the unpaid leave is to commence, except in emergency situations. The request for the unpaid leave of absence shall state the reason for the unpaid leave and the exact dates on which the unpaid leave is to begin and end. Authorization or denial of an unpaid leave of absence shall be furnished to the employee in writing by the Employer. Any request for an extension of an unpaid leave of absence must be submitted in writing to the Employer at least ten (10) days in advance of the expiration date of the original unpaid leave, if possible, stating the reasons for the extension request and the exact revised date the employee is expected to return to work. Authorization or denial of the extension request shall be furnished in writing to the employee by the Employer. Authorization of an unpaid leave of absence under this Section shall not be withheld unreasonably. Any employee on unpaid leave of absence (when the employee is not being paid through payroll) will not accrue benefits (pension, vacation days, etc.), step increases within an established salary range, or any other benefits under this Agreement which are conditioned on service. During an unpaid personal leave of absence, the employee shall be responsible for his or her insurance premiums consistent with Section 16.6 of this agreement and failure to provide the insurance premiums shall result in discontinuance of the employee’s coverage.

  • Unpaid Personal Leave Where an employee has exhausted all paid personal leave entitlements, they are entitled to take unpaid personal leave to care for members of their immediate family or household who are sick and require care and support or who require care due to an unexpected emergency. The employer and the employee shall agree on the period. In the absence of agreement, the employee is entitled to take up to two days (up to a maximum of 16 hours) of unpaid leave per occasion, provided the requirements of 7.2.5(a) and 7.2.5(b) are met.

  • Paid Personal Leave A.   • Payment of Paid Personal/Carer’s Leave (a) If an employee takes a period of paid personal/xxxxx’s leave and meets the notice requirements set out at Clause 44.3 the employer must pay the employee at the employee’s base rate of pay for the employee’s ordinary hours of work in the period.

  • Personal Leave Written request for a personal leave of absence without pay will be considered on an individual basis by the Hospital. Such requests are to be submitted to the employee's immediate supervisor at least four (4) weeks in advance, unless not reasonably possible to give such notice, and a written reply will be given within fourteen (14) days except in cases of emergency in which case a reply will be given as soon as possible. Employees needing personal leave days for appointments with medical practitioners may utilize the personal leave language. Such leave shall not be unreasonably withheld.

  • Personal Medical Leave 1. Accrued 100% sick leave may be used at the employee's discretion. Such leave may be taken before or after the vacation described in No. 3 below.
