Common use of Proof of Death Clause in Contracts

Proof of Death. The Employer and/or the Insurance Company may require proof of death of a Participant or other individual when payment of benefits in respect to the Participant's coverage contingent upon such death. If such proof is not furnished when requested, no benefits will become payable until such proof shall have been received.
