Pulmonary Rehabilitation. Outpatient pulmonary rehabilitation, combined with outpatient cardiac rehabilitation, chiropractic services, and outpatient rehabilitative physical, occupational and speech therapies is limited to 35 visits per calendar year. Prior Authorization is required.
Pulmonary Rehabilitation. Outpatient pulmonary rehabilitation is covered for up to thirty- six (36) sessions per Benefit Period.
Pulmonary Rehabilitation. ♦All patients who complete the Pulmonary Rehabilitation Programme will be given information re exercise programmes available across West Lothian and will be referred to continue exercising through the Exercise Referral Scheme ♦On average 25% of patients who complete the Pulmonary Rehabilitation Programme will continue to engage in exercise, e.g. utilising WLL/ the Pitstop or other leisure facilities, other exercise programmes, e.g. Put Your West Foot Forward and/ or will purchase a WLL/ Pitstop membership The DOLI Project ♦All patients who complete the DOLI Programme will be given information re exercise programmes available across West Lothian and will be referred to continue exercising through the Exercise Referral Scheme ♦On average 25% of patients who complete the DOLI programme will continue to engage in exercise, e.g. utilising WLL/ the Pitstop/ other leisure facilities, other exercise programmes, e.g. Put Your West Foot Forward and/ or will purchase a WLL/ Pitstop membership Xxxxx Xxxxx High School ♦A minimum of 5 Exercise Programmes will be run annually at Xxxxx Xxxxx High School ♦A minimum of 50 new patients per year will engage in the Exercise Programmes ♦At least 20% of patients who complete the 8 week programme will continue to engage in exercise/ purchase a membership at a leisure facility Xxxx 17 Project at St Xxxxx ♦At least 30% of in patients in Xxxx 17 will engage in the supported exercise programme and will utilise the exercise equipment at least twice per week ♦All patients being discharged from Xxxx 17 will be given information re exercise programmes available across West Lothian and will be referred to the Exercise Referral Scheme ♦All patients who engage in the exercise programme whilst an in patient will have an exercise plan incorporated in their discharge plan ♦At least 30% of patients discharged from Xxxx 17 will continue to engage in an exercise programme Process targets ♦100% of new referrals will be contacted by WLL within 2 weeks of receipt of complete referral information ♦95% of new referrals will be seen by WLL and attend for initial assessment within 4 weeks of receipt of complete referral information ♦DNA/ cancellation rate will be less than 10%
Pulmonary Rehabilitation. Services will be provided prior to and following lung transplant, heart-lung transplant and lung volume reduction surgery, and for severe chronic lung disease patients as reviewed and determined by Us. The rehabilitation Services will be covered only when under the continuing supervision of a Physician and in a Hospital environment.
Pulmonary Rehabilitation. This is a covered expense under the Program. Medically prescribed Pulmonary Rehabilitation rendered by a duly qualified Therapist to structure a program of activity, progressive breathing and conditioning exercise, and patient education designed to return patients with pulmonary disease to maximum function. See also Rehabilitation Care Speech This is a covered expense under this Program. Congenital conditions or diseases causing delayed speech development in children are NOT a Covered Expense under This Program. Medically prescribed services of a legally qualified Physician or qualified Speech Therapist for respiratory or rehabilitative Speech Therapy for speech loss or impairment due to an Illness or Injury, other than a functional nervous disorder, or due to surgery because of Illness. Limited to Twenty-Five (25) visits.
Pulmonary Rehabilitation. When preauthorized in writing by Kaiser Permanente, prescribed pulmonary rehabilitation services are covered, subject to the terms and Supplemental Charges in this benefit schedule (e.g. office visits will be subject to section A, hospital care will be subject to section B, skilled administered drugs will be subject to section F, etc.).
Pulmonary Rehabilitation. Pulmonary Rehabilitation benefits are available on a short-term basis. The benefits consist of treatment within a sixty (60) day period per incident of illness beginning with the first day of treatment. The Member's Primary Care Physician must certify that the treatment will result in a significant improvement of the Member's condition within this time period. The treatment must be approved by the HMO's Medical Director.
