Quality Improvement Project. HPSM will provide support, data, and resources for quality improvement projects through the Quality Improvement Department and through the provision of the Medical Director of the Demonstration Pilot and the Director of the Demonstration Project.

Quality Improvement Project. FHS will provide data and resources for quality improvement projects through the CCS nurses and administrative staff, and through the provision of the Medical Director of CCS and the CCS Administrator.
Quality Improvement Project. The Contractor agrees to identify the Department as a source of funding for this project in any communications to the public. The Department may require the installation of a project sign which identifies it as a source of funding as outlined in the requirements and specifications attached to and made part of this contract as Attachment E. 5.
